Este paquete de contenido adicional incluye 20 nuevos niveles para un solo jugador creados por el equipo original de HeXen, así como una gran cantidad de opciones multijugador nuevas.Deathkings of the Dark Citadel fue la primera expansión verdadera que Raven Software había producido.
